Transitioning into Adulthood is exciting -- whether you’re moving out of your parent’s home or going to college -- but like all life transitions, it can be scary, especially when dealing with a chronic health issue.

The best way to deal with that fear is to be as prepared as possible. By making sure you have the right tools and have done enough planning for your healthcare, you’re putting your best foot forward towards a healthy adult life.
